    Chloroquine resistance and malaria

Rare locally transmitted cases in the city of Santo Domingo (Distrito Nacional) and other provinces. Factors that affect local malaria transmission patterns can change rapidly and from year to year, such as local weather conditions, mosquito vector density, and prevalence of infection.

Chloroquine Resistance in Plasmodium vivax. The analysis used to look for loci associated with chloroquine resistance was a linkage group selection scan of recombinent progeny pre- and post-chloroquine therapy. Allele frequencies of SNPs between parental cross lines are shown in black and red. Plasmodium falciparum malaria in Haiti is considered chloroquine susceptible, although resistance transporter alleles associated with chloroquine resistance were recently detected. Among 49 patients with falciparum malaria, we found neither parasites carrying haplotypes associated with chloroquine resistance nor instances of chloroquine treatment failure. This deficiency explains the reduced ability of chloroquine-resistant parasites to concentrate chloroquine, and it suggests that chloroquine resistance is due to a decrease in the number, affinity, or accessibility of chloroquine receptor sites on a constituent of the malaria parasite.

Several medications are available for chemoprophylaxis. When deciding which drug to use, consider specific itinerary, length of trip, cost of drug, previous adverse reactions to antimalarials, drug allergies, and current medical history. Information in these tables is updated regularly.4.

  6. One of the greatest threats to the control and elimination of malaria is the spread of parasites resistant to anti-malarial drugs. Plasmodium vivax resistance to chloroquine CQRPv is difficult to detect due to the low level of parasitaemia among parasite carriers and also to distinguish from relapses to reinfections 3.

Chloroquine is used extensively in malaria endemic areas in Africa to treat the uncomplicated form of Plasmodium falciparum malaria. However, the efficiency of chloroquine has been severely impacted by the recent development of chloroquine resistant plasmodium falciparum parasites. Chloroquine-Resistant Malaria Chloroquine phosphate tablets are not effective against Chloroquine-or hydroxyChloroquine-resistant strains of Plasmodium species see CLINICAL PHARMACOLOGY, Microbiology. Chloroquine resistance is widespread in P. falciparum and is reported in P. vivax. Before using Chloroquine for prophylaxis, it should be ascertained whether Chloroquine is appropriate for use in the region to be visited by the traveler. Chloroquine phosphate or hydroxychloroquine sulfate Plaquenil can be used for prevention of malaria only in destinations where chloroquine resistance is not present see Chapter 2, Yellow Fever Vaccine & Malaria Prophylaxis Information, by Country. Prophylaxis should begin 1–2 weeks before travel to malarious areas.

Discontinued Plaquenil.other options? The alternative to Plaquenil is a slightly different anti malarial called Quinacrine. Plaquenil replaced Quinacrine formerly called Atabrine for anti malarial prophylaxis so now the Quinacrine has to be made up into pills or gel capsules by a compounding pharmacy.
